Objective:
The objective is to assist children with communication, mobility and adapted/specialized equipment that will enhance their growth and skills in personal, educational and social development leading to and increasing opportunities to live a more independent life.
Eligibility:
Children who have Cerebral Palsy and whose family is a current paid member of the Association for one year or longer. (membership year is from September 1 to August 31 and renewal is every September)
Allocation:
The Grants Committee will bring a recommendation to the Board of Directors who will by motion approve or deny* (see appeal) all Grant requests. If necessary, the Cerebral Palsy Association will assist members in applying to potential matching funders. Members who have not previously received funding will have priority.
Please Note Ð that all requests will become invalid after six months from date of approval unless an extension has been requested and granted. Please contact the Program and Membership Director if you do require an extension - 982-4811.
An application form for funding is required. Complete and return it with specific information regarding the item(s) and cost(s). On all requests include GST and taxes if applicable. If requested by the Grant Committee/Board of Directors you may need to include estimates from two (2) different suppliers/companies that distribute the product/equipment needed (example Ð ramps, renovations to a house, adapted computer devices, mobility equipment).
Provide a Cover Letter explaining your particular need or situation. Attach a support letter from one of the following: therapist, doctor, resource/special education teacher, agency or family worker (i.e. SMD, ChildrenÕs Special Services/Family Services).
Printers, adapted/specialized computer equipment (i.e. switches, expanded keyboards) and specialized computer software program are considered for funding. As a rule funds are not awarded for computer monitors, regular keyboards or hard drives however the Board of Directors has made exceptions and will consider the individual merits and needs as outlined in each application and support documents. The Board of Directors is aware that todayÕs computer technology does play a great role in the lives of people with cerebral palsy.
The Program and Membership Director will call to inform you if the funding has been granted or denied. If granted, you will receive a follow up letter explaining the process to access the funds.
